This communication is to inform you that as of December 15, 2023, I am no longer providing medical services at Advanced Dermasurgery Associates.

Upon departure, I will not be in a position to provide patient care. I plan on relocating to a different practice in the future.

Advanced Dermasurgery Associates has custody of your health records. Your medical records are confidential, and a copy can be transferred to another doctor or released to you or another person you designate only through your permission.

If you would like a copy of your records or would like to transfer our medical records, please contact Advanced Dermasurgery Associates for an authorization form. You have options regarding your continuing medical care. If you need medical care or to follow up on a prior appointment, you may contact Advanced Dermasurgery Associates for further assistance and guidance.

I am grateful that I had the opportunity to provide medical care to you. Providing medical care is my utmost priority, and it is a true privilege to be entrusted with the care of another individual.
