Getting Ready for Your First Visit

Discover all of the information you need for your first visit.

We will make every effort to accommodate your schedule. Please let us know if you will be traveling so that we can schedule your surgery appropriately.

Advanced Dermasurgery Associates participates in a large number of insurance plans; however, you will need to verify our participation with your particular plan before you come in for an appointment. If you are on a managed care plan, please confirm that we have received a referral and prior authorization for your visit. As a new patient, you will be asked to bring all information relating to your Mohs procedure visit with Advanced Dermasurgery Associates.

Please familiarize yourself with some of the information provided regarding your Mohs procedure.

What Your First Visit Will Be Like

On your first visit, we examine your skin cancer, review your medical history and answer your questions. We will also review the pathology report of your skin cancer, give you pre-operative instructions, take measurements and photos of your Mohs surgery site for the Mohs procedure.

We always keep your referring physician informed of your treatment and date of surgery.

What to Bring

  1. Completed patient information forms
  2. Name, address and phone number of your pharmacy
  3. Insurance card(s)
  4. Driver’s license
  5. Any previous medical records, especially test results

Arrive 10 minutes early

Please review directions to our building and plan to arrive 10 minutes before your appointment. Please arrive 30 minutes ahead of your appointment if you need to fill out your paperwork at the office.
